COPC PROJECT GUIDELINES

COPC Project (To be developed during Community Medicine Rotation and completed by graduation.)

This plan needs to be written by end of community medicine Rotation, but does not need to be implemented until the end of PGY2.

  1. Select a problem area of interest and develop or define a related community health objective.
  2. Explore and research sources of information about the problem, the epidemiology of the problem, its significance for a given community and the family physician. Be sure to review the current literature and list resources available.
  3. Clearly state the rationale for the proposed project. Identify medical, family, and community risk factors associated with the problem and its management.
  4. Propose and intervention strategy to meet the health objective stated in I. above. Cite potential barriers to implementing proposed interventions and state strategies to overcome these barriers.
  5. Prepare a written COPC plan describing the above activities using the following format (written plan should be done by end of Community Medicine Rotation):
    1. Rationale of Proposed COPC Project
    2. Identify and describe target population.

      Identify and describe target health problem and associated risk factors.

      Identify and describe potential benefits to the community if intervention strategies are successful.

    3. Literature and Resource Review
    4. Describe current literature relevant to problem and cite appropriately.

      Describe current internet sources of information.

      Describe any current research on this health problem.

    5. Proposed Intervention Strategy
    6. Describe steps needed to implement intervention.

      Describe potential barriers to implementation and present plan to overcome barriers.

      Describe costs involved in project and plan to fund project.

      Include a time frame for completion of project.

    7. Proposed Plan to Evaluate/Monitor Success of Project

Include both short-term and long-term evaluation/monitoring plan.

 

 

  1. Implement the Project (To be accomplished during the year following the Community Medicine Rotation; some time will be given during Rural Medicine for project)
    1. Keep notes on how the implementation went.
    2. Make note of successes and of problems.
    3. Suggest ways in which this project could be extended or enhanced.
  1. Prepare Written Summary of Project and Its Outcomes (To be completed before graduation)
    1. Combining information gathered in V above with VI, prepare a final project summary.
    2. Add a brief statement of clinical implications of your projects and suggestions for future interventions with regard to this health problem.
    3. Submit this report for faculty review.
